Required Forms for Varsity Cheerleading

Every participant must turn in the following forms to be eligible to tryout. You will not be allowed to tryout until both of the following forms are filled out and turned in as well as the proof of admission to Tech (for new students) or a copy of a current Tech transcript and proof of a physical by a doctor.
