Communication is vital to the scholarship management system.  In order to facilitate effective communication concerning selection of recipients, changes in criteria, and formation of annual scholarships, we are providing access to various forms.  As new forms become available, they will be added to this listing.

Administering Units use this form to notify the Scholarship Coordinator of updates or changes to individual scholarship criteria.

Annual Scholarship Worksheet .pdf 
Administering Units use this form to notify University Advancement and Scholarship Coordinator of new annual scholarships anticipated to continue for 3 years or more.
