Office Staff
Note: You MUST see an advisor before filling out any of the forms below. These forms must be authorized and approved before any action will be taken.
- Drop/Add Card may be obtained
in the Student Success Center (Advisor's signature is required and must be returned to
the registration help window, Derryberry Hall, Room 162)
- Change of Major- may be obtained
in the Student Success Center (Advisors' signatures are required and must be returned
to the records office, Derryberry Hall, Room 102)
- Readmission After Suspension-
(Advisor's signature required) may be obtained from http://www.tntech.edu/admissions/pdfs/readmit_app.pdf
- Request for Extension or
In-Residence Study at Another Institution-may be obtained from http://www.tntech.edu/records/pdf/extension.pdf
(Advisor approval must be granted before enrolling in the extension course)
- Repeat Card (no signature required) may be obtained
from the Student Success Center or outside of the Records Office in the forms rack
