PROMISSORY NOTE FOR SGA EMERGENCY LOAN

To apply for a Emergency (book) loan for the Fall 2009 semester follow the steps below:

When Applying During Following Office Hours (M-F, 8-2:30)
- Complete online form below entirely and submit
- Wait Two Business Hours and then go to Loan Accounting to sign Prom Note

When applying after 2:30 or on a weekend you may go to loan accounting the following day.
If you fail to fully complete all items (need full addresses on both references) your loan will be rejected and you will be asked to start over.
Loan Accounting is located in the 1st floor of Derryberry Hall.

I hereby apply for a SGA Book Loan in the amount listed above to assist in the payment of my educational expenses while in attendance during the Fall, Spring, and Summer terms of the Academic Year 2009-2010.

In order to assist in processing this loan, I am listing below the names and addresses of two personal references (must have separate addresses) who will be listed as the co-maker of my Promissory Note. (Must be complete addresses.)

I understand that the University may disclose to a credit bureau both the disbursement of this loan as well as the default of this loan. /I agree that any financial aid received after this loan is processed will be used to repay this loan. If this loan is not paid at maturity, I promise to pay a reasonable attorney’s fee for the collection of same, together with all costs incident thereto. All parties and endorsers hereby expressly waive demand, protest and notice of nonpayment.
